00 per year Job description Service coordination is a service that assists a participant in gaining access to needed waiver services, MA State Plan services and other medical, social, and educational services regardless of funding source with a caseload up to 70 participants (at minimum each participant requires 2 face to face visits/year).
A Service Coordinator employed by PA Health Management must be highly skilled at customer service, be compassionate, and have a desire to help others.
The job roles and responsibilities include, but are not limited to:
Complete a person-centered assessment and a level of care evaluation at least annually.
Develop a service plan for each participant and modify it at least quarterly.
Review the participant's needs, goals, and outcomes with the participant and other persons that the participant requests each time service plan is revised.
At least one telephone call or on a need to bases per quarter.
Must complete at minimum at least two face-to-face visits at participants residences per calendar year for each participant (case loads can be up to as many as 70 participants in home county and surrounding counties could be up to 2 hour radius) but more frequent visits may be necessary depending on the participant's status.
Assist participant to find providers for the services that are approved and authorized through the participant's waiver or program.
Work with varying agencies to assist participant to obtain all approved and authorized services requested.
Evaluate if the participant's needs, goals, and outcomes are being met by the service.
Document all work completed in assisting participant in any way within 24 hours.
Assess all other possible funding sources before submitting requests to the waiver programs.
Job Requirements:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
The requirements listed below are representative of the knowledge, skill, and/or ability required.
Bachelor's degree in social work, psychology, or related field, or 3 years' experience in social work field required.
Must be Computer literate and Proficient in Microsoft Office, Teams, and Internet Prior service coordination experience is a plus.
Ability to navigate different databases.
